Established in 2008, the Lifetime Achievement Award honors a local advocate who has made significant contributions in the areas of community medicine, public health, and/or community development. Each of these areas reflect the legacy of our name sake, Dr. Anthony L. Jordan, and the proud mission of Jordan Health to provide high quality primary and preventive healthcare with dignity and respect to all regardless of ability to pay.

Dr. Jordan founded his practice on the trust of Rochester residents in need of healthcare. His caring and committed services restored and preserved the health of individuals, better educated the community, and fostered unity among his neighbors.

At all of our centers, we work every day to maintain this level of individual caring and dedicated commitment to grow a healthy community. Our beautifully crafted Award is given each year to one outstanding individual who has served innumerable members of the Rochester community—and furthered the education and well-being of our neighborhoods through their authentic dedication and commitment to the collective health of our community!

Established in 2010, the President’s Award is given each year to an individual who has served and advocated for members of the Upstate region to further the health, education, and well-being of our neighborhoods. Their authentic dedication and commitment has improved the collective health of our community. Their advocacy and service is a reflection of the legacy of our namesake, Dr. Anthony L. Jordan, and the president of our health center.

Dr. Jordan’s services restored and preserved the health of the individual, better educated the community and created unity among his neighbors. His leadership set the standard for medical practice beyond the practice walls.

Established in 2013, the Partner Award recognizes the critically important roles played by the many organizations that serve the same population as Jordan Health and without whom our patients would not have sustainable, efficient “wraparound” care.

Our partners come from organizations that improve the stability and vibrancy of our community and its residents. The Award encourages an outstanding leader who fosters an inclusive, diverse and responsive organization even when facing challenges to its core mission and past gains.
